Appointment & Prescription Policy
Churchville Veterinary Hospital P.C.

Appointment Policy

Picture
We strive to provide excellent medical care to all of our patients. Our appointments are scheduled consecutively and in a manner that allows us to provide your pet with the personalized care and attention that they deserve. In order to be respectful of the medical needs of all our patients we have updated our appointment policy.
These changes will allow us to better serve you and others in the community.
Appointments are in high demand and your cooperation and consideration is appreciated.

We require 24 hours' notice if you wish to cancel or change your pet's appointment(s)

48 hours' notice is required if you wish to change or cancel your pet's surgical procedure(s)

Late Appointments:  When client's arrive late to their pet's appointment it can disrupt succeeding appointments and impact the care that we are able to provide. We understand various circumstances can arise that may cause pet owners to be delayed and these are always taken into consideration. Arriving 10 minutes late for your appointment may result in a $15 extended appointment fee. Arriving 15 minutes late or more is considered a missed appointment; however, we will always do our best to accommodate you, if we are able. Extended wait times can be expected. In the event that we are unable to accommodate you, you may be asked to reschedule.

Missed Surgeries: Scheduling a surgery for your pet requires significant consideration and preparation. Failure to provide at least 48 hours' notice to cancel your pet’s surgery will result in a $75.00 missed surgery fee. This reflects some of the costs associated with reserving our surgical suite, allocating additional licensed personnel and preparing rescources specifically for your pet's surgery.

Missed/No-Show Appointments: A No-Show appointment is when a scheduled appointment is missed entirely without any notification. A missed appointment is when advanced 24/48 hour notice is not provided. No-Show and missed appointments inconvenience others that are waiting to receive care for their pet(s). We understand emergencies and unexpected conflicts occur. We will take this into consideration for your first missed or cancelled appointment without advanced notice. In the event that a second appointment is missed / no-showed a $25 missed appointment fee will be placed onto your account. 3 or more missed appointments will require a non-refundable prepayment in the amount of an office exam and may result in loss of future appointment privileges.

 Prescription Policy

We require
72 hours notice for all prescription refills.
(Controlled substances may require more time)

An active veterinarian-client-patient relationship (VCPR) is required for ALL prescription medications.
Professional guidelines and State Law indicate that in order for veterinarians to prescribe
prescription medications, food, or pest prevention, your pet must be up to date on their annual wellness exam.

Prescription refill requests are accepted Monday-Friday during normal business hours.
This includes prescription food, heartworm/flea and tick prevention, and prescriptions called into an outside pharmacy.
All requests must be approved by a veteriarian.

Prescription medications CAN NOT be returned
Prescription medications are regulated by the Federal Drug Administration (FDA) and are subject to the same dispensing laws and regulations as human pharmacies. We cannot return or refund any prescription medications.
